In such cases we will provide you such denial in writing. Another licensed health care practitioner chosen by Oak City Vision Center OD PLLC may review your request and your denial. In such cases we will abide by the outcome of that review. We ask that requests for review or copy of medical information be made in writing to Oak City Vision Center OD PLLC, but this is not a requirement. While we usually respond to these requests within three (3) business days, by law we have a short period of time specified by State or Federal law to respond to your request. We may request an additional extension of time in certain situations.

You may then write a statement of your position and we will include it in your medical record along with any rebuttal statement we may wish to include.

These routine disclosures will not be included in your list of disclosures. You are entitled to one such list per year without charge. If you want more frequent lists, you must pay for them in advance at a fee of $200 per list. We will usually respond to your written request (made to Oak City Vision Center OD PLLC) within thirty (30) days but we are allowed one thirty (30) day extension if we need the time to complete your request.
